In support of PPCWNY’s mission, the Case Manager both directly and in the training and supervision of Patient Navigators, screens for and addresses the Social Determinants of Health (SDOH) needs of our patients by connecting them to community resources and tracking the outcomes. The Case Manager also reports on these to PPCWNY Administration, Board of Directors, NYS Department of Health, and grant funders as requested.
ESSENTIAL FUNCTIONS
Works collaboratively with health center staff to identify patients’ needs and barriers to their healthcare that are out of the scope of what PPCWNY can provide
Researches and identifies providers and community agencies to provide services where the health center staff, or leadership identify a gap
Maintains a commitment to the mission and work of PPCWNY with a deep appreciation for and sensitivity to the guiding principles of the organization
Maintains a commitment to quality internal and external customer service for the agency, allied partners, members, and communities at large
Establishes and maintains strong linkages to appropriate networking affiliations
Ensures referral database is maintained, up-to-date and expanded as needed
Provides referral information and in-services to health center staff and leadership as needed
Develops a standard intake form to inquire about patients’ social determinants of health to be piloted in Syracuse and eventually used agency-wide
Provides assessments and referrals for patients in person and/or by telephone
Identifies and assesses behavioral, psychiatric, and other needs of patients that impact their overall health and daily functioning
Makes mandated child abuse reports to the local/state registry as appropriate
Supports and advocates for patients and their families in obtaining needed services
Coordinates services and connects patients with resources identified
Trains, supervises, and supports Patient Navigators in their role to connect patients to needed resources using trauma-informed care practices
Runs regular Patient Navigator team meetings and biweekly supervisions (or more as needed) with individual Patient Navigators
Maintains appropriate records, analyzes, and reports patient and program data to the AIDS Institute Reporting System (AIRS) and as otherwise required by the NYS Department of Health
Ensures proper documentation is written for all patients in accordance with grant directives
Stays current in relevant patient care issues including but not limited to Social Determinants of Health and Trauma Informed Care through training and reading
Helps promote a climate of positive sexual health
Maintains strict confidentiality in accordance with agency policies and HIPAA
Participates on committees and coalitions, as assigned

Must be able to work a flexible schedule including some evenings and Saturdays and travel within the region (Onondaga County to Erie and Niagara Counties)
Performs all other duties as assigned by supervisor for the purpose of ensuring the efficient and effective functioning of the agency
Requirements
Qualifications
Applicants must possess the following qualifications (or equivalent combination of education and experience):
Education—
Master’s degree from an accredited college and/or university in social work or RN degree
Current RN or LMSW license in NY State
Experience—
Two years of direct experience in human services/case management
One year of supervisory experience
Knowledge, Skills, & Abilities:
Professional verbal and written communication skills
Proficiency in Microsoft Office; Electronic Health Record experience preferred
Excellent organizational skills, great attention to detail, and ability to maintain and report accurate records
Ability to set goals and create timelines for work
Must be able to work independently and collaboratively
Knowledge of trauma informed care tenets and practices

Work Schedule
This is a full-time position, 40 hours a week, position. Regular schedule will be Monday-Friday with some evenings and Saturdays.
Compensation
Minimum starting rate is $29.96/hour.
