Job Summary: The Occupational Therapist will assess participants to determine functional levels, cognitive abilities, and therapeutic activity to promote continued community living while also educating to support participants at highest level of function. The OT carries out duties within the compliance of Federal and State rules and regulations.
Education: Graduate of an Occupational Therapy curriculum that is accredited by the Committee on Allied Health Education and the Accreditation of the American Medical Association and the American OT Association.
Experience: Two years’ experience as an OT with a minimum of one year’s experience working with the frail elderly population. One-year supervisory experience.
Number and Type of Employees Supervised (optional): Certified OT Assistant
Licensure, Registry, or Certification Required: Licensed Occupational Therapist in North Carolina. Valid North Carolina Driver’s License and vehicle.

Key Responsibilities:
• Participates in the Interdisciplinary Team’s pre-enrollment assessment as needed, initial assessments within 30 days of participant enrollment, and periodic reassessments as required.
• Completes thorough assessments to deliver appropriate recommendations related to the service request process and appropriate documentation in the EMR per PACE policy.
• Prioritizes and manages acute referral and periodic reassessments regarding participant’s condition including functional status and cognitive abilities.
• Participates as a member of the Interdisciplinary Team (IDT), by attending staff meetings and taking part in participant care planning.
• Communicates any changes in participant condition to team members.
• Develops treatment plans and contributes to inter-disciplinary programs when indicated, sets goals, and provides one-on-one and group interventions.
• Coordinates 24-hour care delivery.
• Provides training and education to caregivers and PACE of the Southern Piedmont teammates to carry out needed services.
• Analyzes chart and intervention history to provide supportive documentation for participant needs and to assist with discharge/transition planning.
• Utilizes critical thinking skills to provide the appropriate care and services as well as recommend cost containment services and equipment.
• Maintains documentation in the electronic medical records per PACE policy.
• Other duties as assigned by supervisor.
