Job Summary:
The Wound Care Nurse primary purpose of the job is to develop, implement, and supervise
day-to-day wound care program of the facility in accordance with established nursing
standards, the policies, procedures, and practices of this facility and the requirements of
current federal, state, and local standard governing the facility.

Major Duties and Responsibilities
• Wound Assessment and Management:
• Conduct thorough assessments of wounds, including identifying wound type, stage, and
any signs of infection.
• Develop and implement individualized wound care plans based on best practices and
patient needs.
• Apply and manage advanced wound care treatments, including dressings, debridement,
and skin grafts.
• Monitor and evaluate the effectiveness of wound care interventions and adjust
treatment plans as necessary.
• Patient and Family Education
• Educate patients and their families on wound care techniques, infection prevention, and
the importance of adhering to treatment plans.
• Provide guidance on proper wound hygiene, dressing changes, and the use of
prescribed wound care products.
• Collaboration and Communication:
• Work closely with physicians, other nurses, and healthcare professionals to ensure
comprehensive patient care.
• Document wound assessments, treatments, and patient progress in electronic health
records (PCC) accurately and timely.
• Communicate effectively with patients, families, and the healthcare team regarding
patient status and care plans.
• Adhere to strict infection control protocols to prevent wound infections and cross-
contamination.
• Ensure the proper disposal of wound care materials and maintain a clean and safe
environment for wound care procedures.
• Stay current with advances in wound care practices, technologies, and evidence-based
guidelines.
• Participate in ongoing education and professional development opportunities related to
wound care and nursing.
Required Qualifications
• Must have a current unrestricted license as a Registered Nurse (RN) or Licensed
Practical Nurse (LPN) in practicing state.
• A nursing degree from an accredited college or university.
• Must pass a criminal background check.
• Current CPR certification
• Long term care experience preferred.
• 2 years RN experience with a focus on wound care preferred.
• Certification: Wound Care Certification preferred.
• Prior experience in wound assessment and advanced wound care techniques
• Possess strong communication and interpersonal skills.
• Must have the patience, empathy, and compassion needed to care for elderly patients.
• Ability to work cooperatively with other healthcare professionals.

Complies with standards of business conduct, and state/federal regulations and
guidelines.
• Always follow appropriate safety and hygiene measures to protect residents and
themselves.
• Maintains confidentiality of protected health information, including verbal, written, and
electronic communications.
• Reports noncompliance with policies, procedures, regulations, or breaches of
confidentiality to appropriate personnel. Reports any retaliation or discrimination to HR
or compliance officer.
• Reports any allegations of abuse, neglect, misappropriation of property, exploitation, or
mistreatment of residents to supervisor and/or administrator. Protects residents from
abuse and cooperates with all investigations.
• Reports any occupational exposures to blood, body fluids, infectious materials, and/or
hazardous chemicals in accordance with facility policy.
• Participates in all life safety and emergency drills and trainings. Fulfills responsibilities as
assigned during implementation or activation of the facility’s emergency plan.
• Reports work-related injuries and illnesses immediately to supervisor.
• Follows established infection control policies and procedures.
• As a condition of employment, completes all assigned training and skills competency.
• Must be able to perform the physical requirements of the position; lift, push, pull, carry
heavy objects. Stand for prolonged periods of time, walk, bend, and stretch.
