unique needs. We are seeking a dedicated Care Coordinator with a valid Minnesota Nursing License
(RN or LPN) to oversee client care and support our growing team of caregivers.
Job Description:
The Care Coordinator will play a key role in ensuring the quality of care provided to clients by
managing care plans, supervising staff, and maintaining communication with clients and their
families. This position requires strong leadership, attention to detail, and a compassionate approach
to caregiving.
Responsibilities:
Medication and Care Management
● Ensure proper administration and tracking of medications.
● Implement and follow through on doctors' orders to maintain high-quality care.
Quality Assurance
● Oversee the quality of care provided to clients, ensuring adherence to organizational
standards.
● Monitor and address changes in clients’ conditions to adjust care plans accordingly.
Administrative Responsibilities
● Complete and manage all required documentation accurately and in a timely manner.
● Develop and initiate individualized care plans for new clients.
Staff Supervision and Training
● Train and supervise new caregivers to ensure compliance with care protocols.
● Conduct RN-led orientation training sessions to align staff with care standards.
Client and Family Communication
● Maintain open and proactive communication with clients' families to address concerns and
provide updates on care.
On-Call and Shift Management
● Provide on-call support to resolve urgent issues or care needs.
● Manage dropped shifts to ensure continuous, uninterrupted care.
Qualifications:
● Active Minnesota Nursing License – Required
● Experience in care coordination, home health care, or a related healthcare field
● Strong leadership and team management skills
● Excellent communication and organizational abilities
● Ability to develop and implement individualized care plans
● Proficiency in completing administrative documentation accurately and on time
● Reliable transportation and valid driver’s license
Preferred Skills:
● Experience supervising and training caregiving staff
● Knowledge of dementia and cognitive care best practices
● CPR and First Aid certification (or willingness to obtain)
